New Patients

We welcome new patients!  Your first visit to Cindy Chow's Chiropody Foot Specialist clinic is a vital foundation for our relationship with you.  Being well-prepared for your appointment will ensure that the specialist has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling.  To understand exactly what to expect during your first visit, please read this page entirely.

Patient health history form

As Ms. Chow is a licensed chiropodist (foot specialist of Ontario), it is required that any person who wishes to be a patient / client of hers that an intake form and a patient contract / consent form is to be completed.  Please note that the intake forms must be read and filled out prior to your initial visit.  We would be happy to email you the forms or you may pick the forms up at our office before then.  Also, you may fill out the paperwork on the day of your visit so long that it is completed prior to your scheduled appointment time; so we suggest you arrive 10-15 minutes early for this.  Each of our clients are seen by scheduled appointments and our receptionist will call to remind you of your booked time one or two working days prior to it. Kindly give us 24 hours notice for any cancellation, change of time, or lateness (lateness meaning missing your appointment by half or more) to avoid a charge.

What to expect

For the initial visit, we provide 45 minutes.  We make sure to obtain important background information, like your medical history, and give you time to get to know us.  You will be educated on your condition, receive a treatment plan and advice.  If there is time, treatment may be given.  If you desire an extended time of 1 hour, then please advise us when you book and to expect to be charged the going rate for the extended visit.

What to bring on your first visit:

  1. any referral doctor notes
  2. list of your current medications if any
  3. footwear of a representative sample you normally wear and any orthotics / insoles you have worn
  4. wear or bring shorts or pants which can be rolled up past the knees
  5. completed Patient Contract & Intake Form (click below)

Our office is located at 4675 Steeles Avenue East, Unit #1C25, Scarborough, Ontario, M1V 4S5, inside Splendid China Mall.  This is our new location since September 2017.
